If any of your information has changed in relation to your address, insurance information, or therapy information, please inform the staff upon arrival so we can update your records. APPOINTMENT DATES/TIMES:
There are legal limits on how many Suboxone prescriptions a doctor can write each month. When you make an appointment, you are assigned to one of only a certain number of slots available at each clinic session. Therefore it is very important that you keep your appointments and arrive on time.

TRANSFERRING FROM ANOTHER SUBOXONE CLINIC:
If you are transferring from another clinic, please let the ARS staff AND Physician/APP know when your current prescription expires. Failure to do so may result in difficulty obtaining your medication.

PRESCRIPTION MONITORING PROGRAM:
ARS searches all available state Prescription Monitoring Programs (PMPs) as required by law in each state.
